    1. How do I get started?
      California Cryobank suggests that you start by selecting a physician to perform your insemination. If you do not have a physician that specializes in reproductive medicine, you may call our Client Services Department at 866-927-9622, and we may be able to refer you to a physician in your area.

      While in the process of choosing a physician, you should review our information packet and donor catalog. These may be viewed online, or we can mail the information to you. In order to help make your donor selection, you may order a wide variety of donor information including short or long donor profiles and donor audio interviews. You may also make an appointment with our donor matching counselor at 877-743-6384.

      Once you have selected your inseminating physician, you must complete the online agreement (including your electronic signature) and make sure the Authorization for Release of Semen (A1) form is signed by your physician and sent back to CBB. You must have the A1 form signed and returned to us before you can schedule shipment of any donor specimen.

    3. How are the semen specimens packaged and shipped?
      We send the specimens in liquid nitrogen dry shipper tanks via Federal Express or local courier. The specimens are guaranteed in the dry shipper tanks for 7 days from the date shipped. Shipments within the United States are sent using an overnight courier service.

    4. What methods of payment do you accept?
      We accept payment by check, money order, cash and most major credit cards including Visa, MaterCard, American Express, and Discover cards.

    5. Will my insurance pay for the specimens? Can you bill my insurance company directly for remibursement?
      Many insurance companies view fertility as an "elective" procedure, and not a "disease" of the reproductive system. Historically, fertility services were not covered by insurance plans; however, we are seeing a positive trend in the coverage of these services by some companies. Please check your insurance policy regarding coverage of these specific services. We do not have the facilities to bill insurance companies directly for reimbursement, but we can provide you with an invoice to assist you in completing the insurance claim form.

    6. Can I perform the insemination at home or can this only be done at the doctor's office?
      The California Cryobank may send your donor specimen to any address specified on your new account form. We do require our clients to be under a physician’s care. Your physician must sign an Authorization for Release of Semen (A1) form before you may purchase any specimen. We recommend consulting with your physician on all matters relating to inseminations, but the decision to inseminate at home is between you and your physician.

    7. Whom do I call to find out if you have received my paperwork?
      Please call our Client Services Department to obtain information about your account status. Our representatives are available Monday through Friday 6:00 a.m. to 6:00 p.m., Pacific Time, at x1142. You may also call our New Accounts Department, Monday through Friday between the hours of 10:00 a.m. and 3:00 p.m. at x1141.

    8. How do I order the Donor Profile and Extended Donor Profiles?
      Our Donor Profiles (2 pages) are available at no cost on the website, or you may order (at no charge) up to 6 short profiles per month by calling our Client Services Department at 866-927-9622. The Extended Donor Profiles can also be ordered online. You may download the profiles from our website or have them e-mailed to you directly. Our Extended Donor Profiles can also be ordered and mailed to you by calling our Client Services Department. Payment is required before the items can be mailed to you. We can deliver via the postal service or you may request Federal Express overnight delivery at an additional charge. Please see our Donor Information Pricing for current fees.

    9. Do you offer audio interviews with donors?
      Yes. California Cryobank does offer donor audio interviews. Audio interviews can be ordered online (audio files are available for immediate download) or by phone (a CD is mailed to you). To expedite delivery, you may order online (for immediate download) or request that your CD be sent Federal Express, at an additional cost. Please see our Donor Information Pricing for current fees.

    10. Do you offer donor photo matching?
      California Cryobank offers two types of donor photo matching consultations to help you match the facial characteristics of the donor. In option one, you narrow down your choices to six donors or less. One of our counselors will compare photographs of your choosing to photographs of these donors to determine which donor most resembles the photographs you sent us. In option two, you tell us the most important qualities you are seeking in a donor, and a donor matching counselor will identify the donors that best meet those characteristics.     11. Do you offer Childhood Photos of donors?
      Yes, California Cryobank does offer childhood photos for selected donors. To see if a donor has a baby photo available, please consult the appropriate column on our donor catalog, or use the baby photo field in our online power search. You may order baby photos individually, or as part of a package. You may purchase and download baby photos online, or have them delivered by mail by calling our Client Services Department at 866-927-9622. Photos ordered online will not be mailed.     12. Do you offer donor essays?
      Yes, we do offer Donor Personal Essays. Our Donor Personal Essays are presented in a question and answer format. Some donors write extensively and others are more direct and to the point. The Donor Personal Essays are available at no charge to assist you in the donor selection process. Donor Personal Essays are available for most California Cryobank donors.

    13. Do you offer open donors?
      Yes, California Cryobank does offer Open Donors. To see a list of Open Donors consult the appropriate column on our donor catalog, or use the open donor field in our online advanced search.

    14. Do you conduct personality tests on donors?
      Yes. In response to client requests for additional information on our donors, California Cryobank offers results (for selected donors) from the Keirsey Temperament Sorter, a widely used personality test.

    15. May I speak with a staff member who has worked with the donor?
      California Cryobank provides Staff Impressions Reports written by California Cryobank employees who have worked directly with our donors. These reports are available at no charge.

    16. Do you offer a sibling registry?
      California Cryobank offers a Sibling Registry for clients who are interested in expanding their family circle by connecting with potential siblings from families who have used the same donor. Clients who have children using a CCB donor are eligible to participate in the Sibling Registry Program. The Sibling Registry is an optional program. There is no annual subscription fee to participate in this program.

    17. Do you offer adult photographs of donors?In order to protect the privacy of our donors, CCB does not offer adult photographs at this time. We do offer multiple programs to help clients match the facial characteristics of a donor. Donor Baby Photos and Facial Features Reports can be purchased on our website or by phone.     18. What method does CCB use to process donor specimens?
      California Cryobank uses Density Gradient Processing to prepare IUI ready donor specimens. While more costly and time consuming, density gradient processing is superior to other methods. It eliminates white cells that are associated with bacteria, viruses and infection. It pre-selects sperm that are morphologically normal, have superior mobility, and have a lower incidence of DNA anomalies. It removes all traces of seminal plasma, thus reducing post-insemination cramping.

    19. How can I find out the shipment status of my package?
      California Cryobank is now working with Federal Express to offer a new e-mail notification and online package tracking service. Clients can receive an e-mail message once their package is shipped, and then track the package on the Federal Express web site. For more information, visit shipping and ordering.

    20. How does CCB screen donors for the Human Papilloma virus?
      California Cryobank screens all of our donors to reduce the risk of sexually transmitted diseases, including HPV, by excluding donors with known risk factors. Each donor is also given a physical examination with particular attention to the genital area for evidence of HPV infection. There are no FDA approved tests for routine screening of semen donors for HPV. CCB only uses tests that have been validated for both accuracy and reliability, and are licensed by the FDA. They are performed in an independent licensed and accredited laboratory. We believe that the use of unproven tests and methods with claims of improved safety constitute false and misleading advertising.

    21. Whom do I notify to report a pregnancy?
      Please contact our Client Services Department at 866-927-9622 to report an outcome of insemination and/or the birth of your child. You may also complete the Outcome of Insemination Report online.

    22. Where can I find books on related topics?
      California Cryobank has created a CCB Book Store, in association with Amazon, in response to client requests for book titles. The CCB Book Store contains photographs and brief summaries of a variety of books. The links will take you directly to the Amazon website where you can purchase each book. (Please be aware that California Cryobank is not recommending or endorsing any particular book).

    23. What type of storage terms do you offer if I want to purchase and store sperm for a particular donor?
      California Cryobank offers storage terms for purchase and store accounts ranging from 6 months to 10 years. Purchase and Store accounts are a great way to ensure that vials from the donor you have selected are available for both current and future family building needs.

    24. Does CCB offer a vial buyback program?
      CCB does offer a vial Buyback Program. CCB clients can now purchase multiple vials knowing you may have future options. Clients must purchase at least three vials and store them at CCB. Vials must have been purchased on or after April 1, 2005. Other restrictions and limitations may apply.

    25. Does CCB offer a vial exchange program?
      CCB does offer a Vial Exchange Program. CCB clients who purchase and store multiple vials at CCB may exchange the vials for those of another currently available donor. Vials must have been purchased on or after April 1, 2005. Other restrictions and limitations may apply.

    26. Do you offer vials for use in Assisted Reproductive Technologies, such as in vitro fertilization?
      Yes, California Cryobank (CCB) does offer vials for use in assisted reproductive technologies. ART vials are available for selected CCB donors. They are produced in both intracervical (ICI) and intrauterine (IUI) specimen preparations.
